Refund request for deposits from exchange

For compliance requirements and to ensure the security of your funds, please provide the following information for your refund request:

KYC verification page photo:

The KYC verification page photo from the third-party exchange clearly shows your full name, date of birth, and ID number. (If the KYC verification page fails to provide the information, submit a daily/monthly statement displaying your verified real name.)

Refund address:

A photo from a third-party exchange shows the deposit address, with the address in plain text.

Withdrawal record photo:

A photo from a third-party exchange shows the transaction hash, withdrawal address, deposit address, amount, and a timestamp.

Third-party exchange account information:

If the exchange account isn’t in your name, please also provide:
Account holder’s ID document and KYC page photo. 
Explain your relationship with the account holder and why they made the deposit. 
Confirm that the withdrawal address you provide belongs to the same account holder.

Notes:
The refund will be sent back to the original exchange. Ensure the exchange can accept deposits.
Double-check that the address you provided is correct to avoid any issues with the refund process.
